Name Restoration

Request a name restoration to your prior name (before union/marriage) in the Petition (JDF 1011) or Response (JDF 1015). 

Or:

  • File JDF 1824 - Motion for Name Restoration in the court where the decree was entered.  And
  • File JDF 1825 - Order. (caption only)

 

Filing Fee

  • There is no filing fee if you file for a name restoration within 60 days after the decree has been signed.

Optional Court Hearing

Sometimes, you can ask the court to issue your decree without going to a hearing.
